• Kinesiology Health Assessment (health syndromes)
• Blood, saliva or urine test referral (if applicable) – for patients own account
• DNA testing referral (if applicable) – for patients own account
• Exercise prescription as applicable

2nd Consultation: Follow Up evaluation / progress report
